What Happens if a Patient Lives Longer than Six Months in Hospice?

Hospice care can continue indefinitely as long as it remains appropriate for the patient’s condition. Medicare hospice eligibility does not expire after an initial six-month period. Instead, the process shifts to a system of recertification to ensure continuous support.

When a patient lives beyond the initial six-month prognosis, the following occurs:

• Recertification Intervals: Medicare requires eligibility to be recertified at regular intervals. After the first 90-day period and a second 90-day period, the patient must be recertified every 60 days thereafter.
• Physician Confirmation: During each benefit period, a hospice physician and the medical director must confirm that the patient still meets the criteria for terminal illness (a life expectancy of six months or less if the disease follows its normal course).
• No Benefit Cap: There is no hard cap or limit on how long a patient can receive hospice coverage. As long as the hospice medical team confirms the ongoing need for care through clinical judgment, coverage continues through successive benefit periods.
• Continuous Services: Throughout this journey, the patient continues to receive full hospice services, including pain management, nursing care, and emotional or spiritual support for both the patient and their family.
