How is a Six-month Life Expectancy Calculated for Hospice Patients?
A six-month life expectancy for hospice is not an exact deadline or a countdown; rather, it is a medical judgment based on clinical indicators. To qualify for hospice care, two physicians—your attending doctor and our hospice medical director—must certify that a patient’s terminal illness life expectancy is likely six months or less if the disease follows its normal course.
Physicians determine this prognosis by identifying specific signs of disease progression, which may include:
- Functional Decline: A significant decrease in the ability to perform daily activities, such as spending more than half of waking hours bed-bound.
- Weight Loss: Unintentional weight loss of 10% or more over a six-month period.
- Clinical Complications: Recurrent infections (like pneumonia or UTIs), frequent hospitalizations, or symptoms that no longer respond to curative treatments.
It is important to understand that patients often live longer than six months. Medicare coverage is structured into benefit periods—starting with two 90-day periods followed by unlimited 60-day periods. Before each new period, we recertify eligibility to ensure care continues as long as the terminal prognosis remains.
